If you would like to request a copy of your medical records, they can be obtained through our ShareCare portal. Please see the links below for additional information and details:

Patient Medical Records Request:  For patients and legal guardians with legal paperwork. You will be required to provide a valid email address and a government-issued ID.

Third Party Medical Records Requests: For healthcare providers, attorneys or if you are requesting the medical records for someone other than yourself and are not the legal guardian. Please use this link to upload your request along with the patient’s authorization.
