A minor may be seen with a legal adult, other than a parent, and our notarized consent form (see below). The parent must be available for telephone consultation at the time of visit. The providers have the right to not treat the patient if the presence of the parent/legal guardian is deemed necessary for treatment.
To maintain confidentiality of our patients, it is our policy not to release any information regarding medical or financial history with anyone without authorization. If you need to request medical records, please complete the Medical Records Release Authorization Form and return it to the office. If you have any questions about obtaining medical records from Dermatology Consultants of South Florida, please call (954) 752-2630 extension 214. Our office will contact you within 48 business hours and process the request as soon as we receive your signed authorization.
