We Are Paperless
We all know that forms and paperwork are an important part of today’s healthcare environment. We have worked hard to make the process as easy as possible for all of our patients. At Dermatology Consultants of South Florida, P.A., all patient intake is done electronically (saving time and trees)! You can even pay your co-pay and/or any outstanding balance from your own personal device.
Scheduling Your Visit
We are always accepting new patients and are the preferred provider for many HMO and PPO plans.
If you are unsure if we take your medical insurance, please call the office as we accept more plans than we can list!
Before Your Visit
Prior to your visit, you will receive an appointment confirmation text or email from us. If this is your first visit, you will also be prompted to complete your new patient intake forms. Clicking the link to confirm your appointment will automatically lead you to complete your patient registration and prompt you through the entire process. It’s easy to fill out, and we will remember your answers for your next visit, so you only have to make necessary changes in the future.
Registering before your visit helps us serve you better and faster when you come into the office. We understand that sometimes it can be difficult to fill out forms on a mobile device or computer, which is why we do offer the option to do it on our iPads in the office. Please arrive at least 15 minutes early to fill out your pre-visit information on our electronic iPads if you cannot complete it before your appointment.

Prescription Refills
Prescription refills are handled during office hours when we have full access to your medical records. Refills are not routinely filled on holidays, weekends, or if you have not been seen for more than six months.

Fees, Payments & Insurance
We participate in many insurance plans. Please be certain to provide our office with completed and signed insurance forms, billing information (insurance card) and all required referral and authorization forms prior to, or at the time of each visit.
For our patients who do not carry medical insurance, we do ask for full payment at the time services are rendered. All elective cosmetic procedures must be paid for in advance.
We accept all major credit cards.

Spa Service Polices
Arriving for your Experience: Plan to arrive 15 minutes prior to your scheduled appointment. This allows time to check in, change and begin your personalized consultation with our spa attendant. You can enjoy our herbal tea or fresh fruit.
Clothing: We suggest that you wear comfortable clothing. You will be given a plush robe and slippers to wear during your visit. Special attention and careful draping for privacy are observed during all body treatments and massages
Personal Valuables: We do provide lockers, however we request that you leave all valuable items at home. This establishment cannot assume any responsibility for lost or stolen personal items.
Children: To ensure safety and comfort of all our guests, we are unable to accommodate young children during your visits to Spa Cosmedica. Children 13 and over are welcome if they have an appointment for services and are chaperoned by a parent or guardian.
Gratuities are not included in the cost of your services and they are left to your discretion.
Cancellation Policy: If you are unable to keep your scheduled appointments, please call 24 hours in advance of your appointment time. This policy allows others to benefit from our specialized services.
Cell phones: In consideration to other guests in session, please, turn off cell phones, beepers or any sound device.
Payment: Cash, checks, and all major credit cards accepted.
